It cannot be used in conjunction with another dental plan, dental insurance, or financing program such as Cherry.
This plan is only honored at McQuary Dental. It cannot be used at any other dental or specialist office.
This plan cannot be used for hospitalization or hospital charges of any kind.

Any unused services within the patient membership are nontransferable to other plan members regardless of the service.
Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
All payments are due in full at the time services are rendered.
